All first consultations not exceeding 1 hour are charged at the rate of R2000-00.
Payment is required on completion of this form and prior to the said consultation. Should it be agreed during or after your first consultation that further legal services are to be rendered, the applicable fee structure will be explained to you. Consultation fees are not refundable in circumstances of the consultation taking place.

1. Acknowledgement and consent
1.1. I, the undersigned, hereby acknowledge and accordingly consent that Godoni Incorporated Attorneys may from time to time, collect my Personal Information, and which Personal Information, Godoni Incorporated Attorneys may collect, store, use, delete, destroy, outsource, transfer or otherwise Process, as the context and circumstances may require and as contemplated in terms of POPIA for the duration of my relationship and mandate with Godoni Incorporated Attorneys, as well as thereafter, as may be required by applicable law.
1.2. I, the undersigned, understand that my privacy is important to Godoni Incorporated Attorneys and that Godoni Incorporated Attorneys will use reasonable efforts in order to ensure that any Personal Information in their possession or Processed on their behalf is kept confidential, stored in a secure manner and processed in terms of POPIA and South African law.
1.3. I, the undersigned, acknowledge that any Personal Information supplied to Godoni Incorporated Attorneys is provided on a voluntary basis and that Godoni Incorporated Attorneys may not be able to comply with its obligations if my correct and up-to-date Personal Information is not supplied to Godoni Incorporated Attorneys.
1.4. I, the undersigned, acknowledge that I shall be entitled to request reasonable access to my Personal Information held by Godoni Incorporated Attorneys, in accordance with sections 23 and 24 of POPIA, as well as the correction, reduction or deletion thereof, as the relevant circumstances may require.

2. Indemnity
I, the undersigned indemnify and hold harmless Godoni Incorporated Attorneys from any action or claim of any nature whatsoever that might be brought against Godoni Incorporated Attorneys as a result of any personal loss, injury or damage arising directly or indirectly from any act or omission on my part relating to or incidental to the failure from my part to honour the above provisions, or otherwise, as the case may be.
3. Declaration
3.1. I, the undersigned, declare that all information that I have provided to Godoni Incorporated Attorneys is true and correct and that I voluntarily provide such information for the purposes set out herein. I further voluntarily consent thereto that Godoni Incorporated Attorneys may Process such Personal Information in order to represent me and perform my mandate.
3.2. I, the undersigned, undertake to inform Godoni Incorporated Attorneys of any change of my identifying information and/or contact details.
3.3. I, the undersigned, confirm that I understand the nature and extent of this consent and further confirm that I intend to be bound by it, in confirmation of which I sign this Consent.
